Registration Form

Teams are either school or neighborhood-based.
All games will be played at Good Shepherd School

U- 6 players should turn 5 by May 1, 2009         
Cut off for U-8 through U16 is July 31, 2009
New this year, we will accept 16-18 year olds (U-19) We may build smaller teams so we will have enough to form a division.

REGISTRATION: Forms may be downloaded from the website (above) or picked up at the following locations: 

• Aptos Academy, 1904 Bonita Dr. (San Andreas/Larkin Valley exit from Hwy 1)
• 101 Seton Way ( side door please) - Westside Santa Cruz
• Rogue Soccer Shop -  Soquel Drive ( across from Dominican Hospital )
• Jerry’s Sports – 7000 Soquel Ave.

FILL OUT FORM - make sure to include all information and parent signature (be sure to sign in both places)

ENCLOSE: a copy of the birth certificate (for new players) and a check for the appropriate amount (see below). 

COST:       Registrations received between October 15 and October 31: $85 per player
Registrations received between November 1 and November 30: $95 per player

                  Registrations received after December 1: $110 per player

Scholarships available to qualified applicants.

REFUND POLICY: All refund requests must be in writing or by email.  Requests received before March 7, 2009 will be refunded, minus a $20 registration fee. THERE ARE NO REFUNDS AFTER MARCH 7.

DATES: Spring soccer season runs from March 7 to May 16. There are no games on April 11. Coaches will be contacting their teams after Feb. 15.
